comprehensive benefits package is offered to benefitted
employees, consisting of various Health/Vision Plans, Dental
Plans, Life Insurance, State Disability Insurance, and
Retirement Benefits. Other benefits include:
401(a) & 457 Deferred Compensation Plans, Additional Life Insurance,
Flexible Spending Accounts (Health and Dependent Care) and Long
Term Disability insurance (LTD). Please review your

As a new hire or promoted (non-benefitted to benefitted) employee, you have 30 days
from your date of hire or promotion to submit most benefit
enrollment requests. Employees MUST enroll in benefits using the
Employee Online
system. Prior to beginning the online enrollment process, please review
the
Employee Online Benefits Guide for detailed information on enrollment into the following
benefit plans:

Beneficiary Designation |
All benefitted employees are automatically enrolled in basic life insurance and in the CalPERS retirement system.
To designate beneficiaries for these benefit plans, please submit the following
benefit forms to the Human Resources Benefits Division within
30 days of hire or promotion date.

Other Benefits Enrollment
|
|
The City offers Other Benefits including Additional
Life Insurance, 401(a) & 457 Deferred Compensation Plans, and State
Disability Insurance to newly hired or promoted employees. To enroll
in Additional Life Insurance or a Deferred Compensation Plan, please
complete the forms listed below.
For State Disability Insurance, enrollment is AUTOMATIC
for General Unit (SEIU), Refuse (SEIU) and
Confidential employees. NO enrollment forms are necessary.

*Enrollment forms for the 401(a) Deferred Compensation
Plan MUST be submitted within
30 days
of hire or promotion date.
**Enrollment for the 457 Deferred Compensation
Plans and Additional Life Insurance can
be submitted at anytime during the year.
For Additional Life Insurance enrollment requests outside of
the initial 30 day
window from hire or promotion date, a
Medical History Statement must be completed. |
