This is a City-paid life insurance plan through Standard Insurance and is available to regular, full-time employees only. The values of the life insurance may differ between bargaining units. Additional coverage may be available through the respective associations.

1. Enrollment is automatic for full-time benefited employees.
2. Employees should submit the beneficiary designation card within the first 30 days of employment.
3. Employees may change beneficiaries at any time by submitting a beneficiary designation card.
