insurance coverage is available to all benefitted employees and
eligible dependents; for more information please review the
brochures to the right.
- Enrollment is automatic for benefitted employees.
- Employees should submit the beneficiary designation card within the first 30 days of employment.
- Employees may change beneficiaries at any time by submitting a beneficiary designation card.
