This is a DHMO dental plan.  Participants may only go to dentist with in the Local Advantage Plan.

There is no annual deductible.

The maximum benefit paid per calendar year is $2000.00 per person.

Dependent children are covered until age 23 if a full-time student or 19 years of age if not a full time student.

Each family member may sign up with a different dentist.

Orthodontic Treatment: Paid at $1,250 Discount from UCR (Adults and Children); Lab fees $220.
