This is a PMI/DHMO dental plan.  Participants may only go to dentist with in the Delta Care PMI/DHMO list.

There is no annual deductible or annual dollar maximum.

Dependent children are covered until age 24 if a full-time student or 19 years of age if not a full time student.

Each family member may sign up with a different dentist.

Orthodontic Treatment: Start-up fees (excluding records $350.00). Dependents children to age 19 $1600.00 and Adults and covered full-time students $1800.

The Deltacare enrollment packet contains complete details of benefits, coverage areas, and exclusions. The City of Riverside Human Resources Department and Deltacare Member Services are also available to answer questions.
